The 2021 procedure coding system (NCMP-NCSP-NCRP) issued in Norway

In early November 2020, the final version of the procedure coding system for 2021 was issued by the Norwegian Directorate for e-Health. The coding system consists of three parts: NCMP for medical procedures, NCSP for surgical, and NCRP for radiological procedures. The following documents are available:

  • All procedure codes 2020-2021
  • Changes in NCMP 2020-2021
  • Cumulative Changes in NCMP 2006-2021
  • Changes in NCSP 2020-2021
  • Cumulative Changes in NCSP 1999-2021
  • Changes in NCRP 2020-2021

The following changes were made in the surgical procedure coding system (NCSP):

  • Two new surgical procedure codes introduced for laparoscopy:
  • JCE31 “Laparoscopic surgery for oesophageal atresia or congenital tracheoesophageal fistula”
  • JGB21 “Laparoscopic partial recto-sigmoidectomy and abdominoperineal pull-through anastomosis”
  • Two new categories added for knee ligament procedures - for primary reconstruction and revision reconstruction, and 16 new codes added
  • New procedure code NAT27 “Posterior correction of the thoracolumbosacral column with ilium fixation, with an internal instrument”

Two new radiology procedure codes (NCRP) were introduced:

  • PJK40K “Marker insertion in the axillary lymph node, ultrasound-guided” - a new procedure for neoadjuvant treatment of breast cancer
  • SAC0AK “UL Neuromuscular ultrasound” for investigation and follow-up of neuromuscular disease

Nine new medical procedure codes added (NCMP):

  • AXDP05 “Video Head Impulse Test (vHIT)”

High speed video for measuring vestibular reflex during rapid head movements. Including interpretation of video recordings. Note: Introduced at the request of Helse Bergen

  • JAGD55 “Patient-administered peritoneal dialysis”
  • MAFX11 “Lactate measurement in the fetal scalp”
  • WO6100 “Preparation of a new (re) habilitation plan”
  • OCAA00 “Revision of (re) habilitation plan”
  • RXGD50 “Initiation of patient-administered hemodialysis”

Including: training in the use of medical technical equipment Note: There is already a corresponding start-up code for patient-administered peritoneal dialysis, and it is natural that there is also a corresponding start-up code for patient-administered hemodialysis

  • RXGD55 “Patient-administered hemodialysis”
  • WABA30 “Insulation due to radiation protection considerations”

Note: New code for isolation measures for use when the patient has been treated with radioactive medicine or has been exposed to radioactive exposure that may endanger the environment.

  • WMAA11 “Interdisciplinary assessment of known condition”. New code for reporting interdisciplinary assessment when the patient has a known diagnosis and is followed up interdisciplinary.

The report proposes a novel type of “early method evaluation” to complement existing national and mini-HTA frameworks in Norway. This new assessment type can be performed before classical HTA. Early Method Evaluation can be used locally at department or hospital levels when new technology/innovation is tested to analyze the impact on patient, staff, finances before reliable evidence from clinical studies became available.

In late October 2020, the BfArM (Federal Institute for Drugs and Medical Devices) has published the final version of the OPS 2021 (Operation and Procedure Keys) system. The OPS, together with the ICD-10-GM (International Statistical Classification of Diseases and Related Health Problems, 10th revision, German Modification), forms the basis for the reimbursement systems in outpatient and inpatient care.

In October 2020, a completed mini-method assessment for the coronary sinus reducer stent (Neovasc) implantation in refractory angina pectoris treatment was published within the New Methods framework. The Oslo University Hospital carried out the evaluation. The technology was recommended for routine use in hospitals.

In late September 2020, the Austrian version of the DRG system (LKF) 2021 model with the relevant supplementary documents was published in Austria. For 2021, the main changes include the addition of sixteen new procedure codes and the introduction of two new procedure codes for billing in the day-case settings, update of cases flat rates, and adjustments of the cases daily component calculation.

In this framework, all innovations should undergo HTA before being funded. In August 2019, the Norwegian Institute of Public Health has released an HTA report regarding the effectiveness of treatment for localized prostate cancer commissioned by the Centre for joint decision-making to summarize the findings regarding this method. The effectiveness of the several treatment methods is unknown due to the lack of evidence. There is little or no difference in survival after radical prostatectomy compared to low-dose-rate brachytherapy.

It was outlined that it is uncertain to what extent Prosigna contributes prognostic information that turns into better clinical results; conclusions about the cost-effectiveness of Prosigna cannot be made due to the lack of existing information.

Although the mobile stroke unit care increases the number of patients who receive thrombolysis, its efficacy is unknown due to the lack of evidence.

In June 2019, the Norwegian Institute of Public Health (NIPH) has released an HTA report for TAVI in patients with severe aortic stenosis and intermediate surgical risk commissioned by the National System for Managed Introduction of New Health Technologies within the Specialist Health Service in Norway. The cost-utility analysis indicated that TAVI was slightly more effective and more costly than open surgery. These findings can help decision makers appraise the intervention against the official priority setting criteria in health care sector applicable in Norway.

On the 7th of May 2019, the new evaluation for bone-anchored prosthesis has been commenced in a framework of New Method, which provides a way for a managed introduction of innovations into Norwegian health care system. The proposal for the assessment was submitted by Oslo University Hospital in order to receive an opportunity to offer this method to achieve overall money saving and medical benefits as nowadays, only conventional prostheses are offered, and patients are sent to Gothenburg to receive the treatment, that is financed by the Norwegian health service.

The results have shown that the efficacy and safety evidence of the procedure is too uncertain for now, and its economic evidence is also very limited. On the 14th of August of 2018, the Norwegian Institute of Public Health (NIPH) has published a health technology assessment report in relation to autologous hematopoietic stem cell transplantation (AHSCT) comparing with standard treatment for patients with systemic sclerosis. The results have shown that the transplant‐related mortality after AHSCT is high, but risk of organ failure and death is lower after two years follow‐up and beyond compared to standard treatment, AHSCT provides a significant improvement of skin involvement and lung function measured by forced vital capacity compared to standard treatment with cyclophosphamide injections.

In June of 2018, the Norwegian Institute of Public Health (NIPH) has published a single technology assessment regarding the MicraTM Transcatheter Pacing System (Micra TPS) - a leadless pacemaker produced by Medtronic to reduce the rate of complications following pacemaker implantations through design and novel technology. The objective of the assessment was to investigate the clinical efficacy, safety and cost effectiveness of Micra TPS in patients indicated for single chamber ventricular pacemaker implantation. The results have shown that the current evidence is not sufficient to prove that the Micra TPS gives less complications than standard pacemakers, also the use of Micra appeared not to be cost-effective.
