The Hybrid DRG reimbursement agreement for 2025 has been approved in Germany

On December 19, 2024, the Hybrid DRG reimbursement agreement for 2025 was published. It determines which interventions will be reimbursed via a hybrid DRG mechanism and the associated tariffs. The newly introduced DRGs mainly belong to the endoscopy, gastrointestinal, and men’s health fields.

  • The key changes introduced in the 2025 agreement are provided below:
  • The list of OPS codes eligible for grouping into Hybrid DRGs was significantly expanded (338 OPS codes in 2024 vs. 575 in 2025);
  • The number of Hybrid DRGs increased to 22 (12 in 2024);
  • The following groups of procedures are covered: arthrodesis of the toe joints, certain hernia operations, anal fistula operations, gallbladder, liver, pancreas operations, testicular and epididymal operations, removal of ureteral stones, excision of a pilonidal sinus, lymph node biopsies, and ovariectomies;
  • Two tariffs are provided: without and with postoperative treatment in the hospital.

The examples of the newly introduced Hybrid DRGs are provided below:

  • G26N “Hybrid DRG of DRG G26A Other interventions on the anus or anoproctoplasty and reconstruction of the anus and sphincter in the case of anal fissures and hemorrhoids, age <18 years or with a complex diagnosis or with minor intervention on the rectum”;
  • H41 “Hybrid DRG of DRG H41D Other complex endoscopic retrograde cholangiopancreatography (ERCP) or specific endoscopic procedure without specific malignant neoplasm”;
  • M04M “Hybrid DRG of DRG M04D Interventions on the testicles without extremely severe complications (CC), without specific intervention, without moderately complex intervention or age >2 years, without severe CC or without bilateral undescended testicles or age >13 years”.
