The draft release of the DRG package for 2027 was published in the Netherlands

On December 19, 2025, the Dutch Healthcare Authority (NZa) published a draft release of the DRG package for 2027 (RZ27a), which applies to specialist medical care. 

The update focuses on aligning the DRG structure with current clinical practice by adding and updating procedure codes and revising existing DRGs. Several outdated or overly specific procedure codes will be replaced with broader, technique-neutral definitions, and transition measures will be included to support ongoing care pathways.

Seven new procedure codes, one new diagnosis code, and one new supplementary payment (OZP) across in-vitro diagnostics, obstetrics and gynecology, gastroenterology, and urology are expected to be implemented. The other changes include technical changes and tariff updates. 

One of the most substantial changes is the restructuring of complex care, particularly in gastroenterology. Endoscopic procedures in gastroenterology will be grouped into three levels of complexity (simple, complex, and super complex), with DRGs restructured to reflect differences in intensity and length of stay.

The final release of the DRG package for 2027 (RZ27a) is expected to be published by the NZa no later than May 1, 2026. The effective date of the release is January 1, 2026.
