Draft versions of new Italian classification systems for procedure and diagnosis coding released
The Ministry of Health is currently working to update classification systems for procedure and diagnosis coding in healthcare information systems.
From January 1, 2027, clinical data contained in the hospital discharge form (SDO) and other information flows will be coded using the following classifications:
- International Statistical Classification of Diseases and related health problems, Italian version (Italian modification) of the International Classification of Diseases - 10th revision (ICD-10-IM), for coding primary and secondary diagnoses;
- Italian Classification of Surgical Procedures and Interventions (CIPI), for coding primary surgical interventions or deliveries, and secondary surgical interventions or diagnostic and therapeutic procedures.
On February 13, 2026, the Ministry of Health published webpages dedicated to the “NSIS-CLASS” project, which aims to govern the transition of national health information systems from the ICD-9-CM standard.
Before the formal adoption of the ICD-10-IM and CIPI classifications, the Project foresees an experimental period, which began in spring 2025 and will continue with the fundamental contribution of all the Regions and Autonomous Provinces and the structures operating in the territory.
- During Phase 1, the Ministry of Health coordinated a training and improvement program for classifications and related products, involving representatives from the Regions and pilot facilities, and envisaging the activation of a "cascade" information dissemination mechanism;
- During Phase 2 of the trial, which is currently ongoing, the Ministry of Health will proceed to carry out further refinements to the experimental versions (so-called "Gamma") of the classifications and related products, receiving any feedback.
The technical documentation of the testing versions currently includes systematic lists, transcoding tables (from ICD-9-CM) of ICD-10-IM and CIPI classifications, and the Guide for CIPI classification.
The final version (so-called “Delta”) of the ICD-10-IM and CIPI v. 2025 classifications will be released by the end of 2026.
Starting in 2027, the classification systems are expected to be updated at least every two years, by decree of the Ministry of Health.
