2026 “Low variability care” system (APR-DRG) released in Belgium

In January 2026, the National Institute for Health and Disability Insurance (INAMI-RIZIV) updated “Low variability care” (APR-DRG) system for 2026. This system was introduced in 2019 for certain low-cost and non-complex procedures reimbursed via APR-DRG groups instead of individual physician fees. Unlike in other countries, the DRG tariff does not cover materials and ward stays.

Overall, no significant changes were made to the system:

  • No new DRGs were added;
  • The total number of DRG remains unchanged from 2025 (61);
  • On average, DRG tariff changes from 2025 to 2026 were close to zero (mean -0.5%), with a few individual DRGs showing both increases and decreases.
